日本における排尿が自立していない児を対象とした採尿方法に関する看護師の実態調査

要旨

小児科および新生児科の看護師による採尿の実際や工夫の調査を目的とした.独自の無記名自記式質問紙を作成し,超低出生体重児から3歳児以下に採尿経験を行った経験のある看護師308名に調査を行った.倫理審査委員会の承認を得た.採尿の工夫の1位は,超低出生体重児が「綿球やガーゼに染み込ませ絞る」(27.3%),極低出生体重児が「採尿バッグの粘着面を小さくカット」(26.9%),低出生体重児が「採尿バッグ内に空間を作る」(28.6%),乳児が「通常の採尿バッグの使い方」(39.3%)であった.乳児や幼児の「綿球やガーゼに染み込ませ絞る」工夫は比較的多かった(23.0%,21.6%).1回で採尿できないことを負担に思う看護師は多かった.綿球など根拠の確立していない方法でも,児の負担軽減のために用いていた.看護師および児の負担を軽減できるような採尿方法の検討の必要性が示唆された.

Abstract

The aim of this study was to investigate the practices and techniques used by pediatric and neonatology nurses for urine collection. A unique, anonymous, and self-administered questionnaire was created and administered to 308 nurses with experience in collecting urine from extremely low birth weight infants to children under three years of age. Approval was obtained from the ethics committee. The most common urine collection techniques were“squeezing cotton balls or gauze with urine”(27.3%) for extremely low birth weight infants,“cutting the adhesive side of the urine collection bag into a small size”(26.9%) for very low birth weight infants,“creating space inside the urine collection bag”(28.6%) for low birth weight infants, and“using a regular urine collection bag”(39.3%) for infants. In infants and toddlers,“squeezing cotton balls or gauze with urine”was relatively common (23.0% and 21.6%, respectively). Many nurses found it burdensome that urine could not be collected on a single attempt. Even unproven methods, such as cotton balls, were used to reduce the burden on infants. These results suggest the need to consider urine collection methods that can reduce the burden on nurses and children.

要旨

背景:創傷滲出液中のマクロファージ由来の老化関連分泌表現型(SASP)因子が治癒遅延に関与する可能性があるが,線維芽細胞との相互作用は不明である.本研究は老化マクロファージが線維芽細胞の創傷治癒に及ぼす影響をin vitro共培養スクラッチアッセイで検討した.方法:750μMの過酸化水素水でマウス骨髄由来マクロファージを老化誘導し(H2O2処理BMDM),老化関連遺伝子および上清中SASPタンパク質を測定した.線維芽細胞をH2O2処理BMDMと共培養し,創傷治癒と老化関連遺伝子発現を解析した.結果:H2O2処理BMDMではCdkn1a等の発現が増加し,上清中のTNF-αおよびPAI-1が上昇した.H2O2処理BMDMとの共培養下で線維芽細胞の創傷治癒は遅延し,Tnf,Il1b,Sod1発現が増加した.結論:老化マクロファージは線維芽細胞の創傷治癒を遅延させ,SASP因子の関与が示された.

Abstract

Purpose: Hard-to-heal wounds impair patients’ quality of life. Senescence-associated secretory phenotype (SASP) factors from macrophages in wound exudates may contribute to delayed healing, but their interaction with fibroblasts remains unclear. This study examined whether senescent macrophages impair fibroblast wound healing using an in vitro co-culture scratch assay. Methods: Mouse bone marrow-derived macrophages (BMDMs) were treated with 750μM hydrogen peroxide (H2O2) to induce cellular senescence. Senescence in H2O2-treated BMDMs was evaluated by RT–PCR analysis of senescence-related genes and SA-β-Gal staining. Protein levels of SASP-related factors in the supernatants of senescent BMDMs were quantified by ELISA. H2O2-treated BMDMs were co-cultured with fibroblasts using an insert-based system, and fibroblast wound healing and gene expression were evaluated. Results: H2O2-treated BMDMs showed increased expression of Cdkn1a, Serpine1, Cxcl2, and Sod1 and a higher proportion of SA-β-Gal–positive cells. Protein levels of TNF-α and PAI-1 were significantly elevated. Fibroblast wound healing was significantly delayed at 24–72 h. Fibroblasts co-cultured with H2O2-treated BMDMs exhibited increased expression of Tnf, Il1b, and Sod1. Conclusion: Senescent macrophages impaired fibroblast wound healing, suggesting that macrophage-derived SASP factors may contribute to delayed wound repair.
